Root Cause: 
Human error, our operator’s & inspections were at fault as they didn’t follow the YACL-WP PD 99-01 on this batch of 17 coveralls.
Corrective Action : 
Retraining to all those involved in any future Cll1 – coverall production. and always make sure to meet the customer requirements..
Before any future production of the CLL1 – coveralls the below will be followed. Every machinist – Sewing, eyelet & over locker. Each press stud & metal eyelet operator. Each Inspector will go through re training of the YACL-WP PD 99-01 Each operator will inspect their own work before passing to an inspector. Each pocket over locked will be 100% inspected prior to being sewn onto a CLL1 – coverall. All Velcro patches will be inspected for broken stitches. Each press stud will be inspected by the operator to make sure it is formed correctly in accordance with YACL-WP PD 99-01 Only when the CLL1 – coverall is 100% correct will the QC stamp be applied. Production aids – visual production examples for each station involved in CLL1 – coverall production will be signed by all involved. Prior to dispatch at least one Director will re inspect boxes at random. Back up will be fully trained in all areas of the CLL1 – coverall production to cover holidays & sickness.
